A third premature infant has died after being given an adult-sized dose of heparin, a blood thinner medication.

Two girls less than a week old died earlier at Methodist Hospital's Neonatal Intensive Care Unit in Indianapolis, Indiana. Heparin is often used to prevent blood clots that could clog intravenous tubes.

A gifted lawyer whose life plunged into a "downward spiral" after being misprescribed medication for a minor ailment today won a High Court battle against the dispensing pharmacists.

Cathy Horton, described by a judge as a "woman of many accomplishments" suffered paranoia, depression and psychotic episodes after taking steroid tablets eight times the strength of her normal dose.

Handwriting samples of 209 health care professionals, including 82 doctors, were studied.

The handwriting of doctors was no less legible than that of non-doctors. The sloppiest writing was associated with being a male executive.

A pharmacy erroneously made a drug 10 times more potent than intended, which killed three people who received it at an Oregon clinic, the state medical examiner said Friday.

ApotheCure Inc., a drug compounding pharmacy company in Texas, said an employee made a weighing error in the creation of the drug colchicine.

electronic Rx now legal in Georgia

Physicians and pharmacists in the State of Georgia are now legally allowed to transmit prescriptions electronically. In August, 2006, the Georgia State Board of Pharmacy adopted new e-prescribing rules that officially went into effect on September 7th.

Utah mother catches dosage error

Becky Vanderlinden was surprised last week when she looked at the dose instructions on the bottle of antibiotics for her toddler. Vanderlinden had called the pharmacy to check the dosage and was told it was correct. But the label was wrong.
